How to go about informing pediatricians that you would like your baby to be seen by them?

I am 33 weeks and I will be delivering my baby in a different city than the pediatrician that I have chosen is in. They said that they would just have the on call pediatrician at the hospital I am delivering in see the baby for the initial checks after the baby is born and that pediatrician would discharge he baby to home. After the discharge I was planning on having a pediatrician that is a little closer to home see my baby for the rest of the check ups. I was just wondering how I go about informing the pediatrician I plan for my baby to see that I want her to be the pediatrician. Do you just call the clinic and let them know that you are delivering soon and you want them to be the pediatrician or is there more to it?
